Union City Health and Rehabilitation had the largest capacity among nursing homes in Obion County for the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows.
The facility reported 115 available beds, which was 31 more than Obion County’s average for the period. An average of 68.2 residents received care at the facility during this timeframe.
The CMS gave Union City Health and Rehabilitation a 1 out of 5 overall rating. This falls below the Tennessee average for nursing homes, which was 3.
For the same quarter, The Waters of Union City, LLC was second largest with 80 beds, and Obion County Nursing Home ranked third with 56 beds.
Union City Health and Rehabilitation recorded $15,920 in fines and one penalty as of the first quarter of 2026.

The U.S. Census Bureau estimates that as older American populations continue to increase, younger groups are declining. From 2020 to 2024, the number of people age 65 and older rose by 13%, while those under age 18 decreased by 1.7%. Over the past 100 years, the older population has rapidly expanded, growing from 4.9 million in 1920 to above 61 million by 2024.
A 2021 analysis indicates that nearly half of Americans in this age range, or about 28 million people, could need long-term services and support. Projections also suggest more than a third may ultimately require care in a nursing facility.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
